Cost of Care
We accept four insurance plans:
- Aetna-Trinity (Mt Carmel Employee Plan
- Traditional Medicare (with NO Advantage Plan/Replacement Plan)
- MediGold (Medicare Advantage Plan)
- Tricare
We accept HSA and/or FSA cards for payment. All other plans are out of network, and we operate with a traditional fee for service at time of appointment. New Patient Forms
We ask all incoming patients to please fill out a records release form. This helps Dr. Christofides get a clear picture of your medical history for the best care possible.
