Endocrinology Associates Notifies Patients of Potential Security
Incident Involving Personal Health Information
Endocrinology Associates is committed to maintaining the
security and privacy of the personal health information of its patients. Despite this commitment, we are notifying our
patients of a potential security incident involving the personal health
information of our patients.
We are currently in the process of renovating our property
at 72 W. 3rd Ave. During this
renovation, we are storing patient charts in a rented POD located on site. On the mornings of June 15th and
June 19th, we noticed the POD’s padlock had been removed. Police were notified. No further incidents have occurred.
We conducted an inventory of the POD confirming that no
personal health information was missing.
While we cannot confirm with certainty, we do not believe based on our
inventory and the physical condition of the POD any charts were opened,
reviewed, or copied.
As for the content of the physical charts, we do not
maintain financial information of our patients in the charts. However, some charts did contain social
security numbers. To date, we have not
received any indication, notice, or response from any patient that their personal
health information has been stolen or compromised in any fashion.
We notified our patients by mail of the situation. We request that any patient report directly
to us if they are aware of any information concerning the improper use or
access of their personal health information.
We continue to monitor this situation closely. In addition, we have implemented enhanced
security measures to ensure this situation is not repeated. We appreciate the commitment and loyalty of
