We will be happy to continue your care; however, we will no
longer file your insurance claims. We will
provide you with an itemized receipt at the end of your appointment which you
can use to submit your claim to Medical Mutual.
As per our office policy, you
will be responsible for payment at the time of service.
If you choose to seek another physician who is covered by
your plan, we will be happy to transfer your records. You will need to complete a Release of
Information form. Once you have
completed it with the contact information of the physician to whom you want us
to send the records, please sign the form.
You may send the form to us via e-mail, fax, or US Mail, and we will
begin the transfer process.
