Dear Patients:
We want to ensure a safe environment for all of our patients and staff so we are implementing the following policies immediately:
- We are happy to change any scheduled visit to a virtual/telemedicine appointment using GoogleDuo or FaceTime. Please contact the office to arrange this change. The doctor will ask you for recent weight, blood pressure and pulse readings, so please be prepared to provide this information. All telemedicine appointments are billed at the same rate as in-person visits.
- All in-person visits to the office are By Appointment Only!
- You must be wearing a mask to come to our building for an in-person appointment. The mask can be homemade.
- We ask that only the scheduled patient comes into our building for his/her appointment. No visitors allowed!
- If you have recently traveled outside of Ohio, please call our office to change to a virtual appointment or reschedule after a 2 week quarantine period.
- Please call from our parking lot when you arrive so we can let you know that an exam room is available for you to go into immediately. We want to minimize patient interactions as much as possible and give our staff adequate time to properly disinfect exam rooms between patients.
IF YOU ARE SICK, HAVE A FEVER OR A NEW COUGH, DO NOT COME TO OUR OFFICE. CONTACT YOUR PRIMARY CARE PHYSICIAN IMMEDIATELY FOR INSTRUCTIONS AND LET US KNOW SO WE CAN RESCHEDULE YOUR APPOINTMENT.
